Please check the following as confirmation of acceptance:

- The professional, Katie Osborn, is hired as an independent contractor and understands that they are responsible for filing the appropriate taxes

- A final artistic video to be used in conjunction with the singers of the sINgSPIRE Virtual Choir

- All arrangements for filming will be made and enacted by the hired.

- A final cut of the video will be sent to Breathe Bravely no later than September 6, 2021.

- Ashley Ballou-Bonnema is the main contact for this contractual agreement and all collaborative meetings and assigned specifics will be set between she and the hired.

- Katie Osborn and all elected individuals creatively involved will be given artistic credit.

- If the contractual terms are not met, Breathe Bravely reserves the right to forfeit the contract.

- Compensation for the work provided will total $1,000 USD. Partial payment will be sent upon signing of this document and the remainder will be sent following September 6.

I, the undersigned, do hereby grant or deny permission to Breathe Bravely to use the image and/or recording of the above named artist for creative, promotional, informative (social media), and educational purposes. I acknowledge that I own the rights to any photo or video shared with Breathe Bravely for use in Virtual Choir video production, and media/promotional releases, and I give my permission for Breathe Bravely to use aforementioned creative contributions in a public manner as respectfully seen fit. *
